Standard spirometry can look normal in people who are genuinely breathless, because it measures how fast air moves rather than how well oxygen crosses into the blood or how the lungs behave under exertion.
Common causes of chronic cough include asthma, postnasal drip, acid reflux and certain medications; breathlessness can come from the lungs, the heart, deconditioning, or a combination. A structured workup checks each possibility in turn rather than guessing.
